Mary Rutan Foundation Medical Careers
2008/2009 Scholarship/Loan Application

The Mary Rutan Foundation Board is pleased to offer Logan County residents an opportunity to apply for medical career scholarships/loans for the 2008/2009 academic year.

Eligibility: A student who is officially accepted into a medical career program of choice. (ie: medicine, nursing, pharmacy, allied medical)  If awaiting approval letter for acceptance, please contact the Foundation office at 937-599-7003 for eligibility confirmation.  An applicant enrolled in PRE: Medicine, Nursing, or Allied Medicine would NOT be eligible. 
Instructions For Completing & Submitting Application:  Submit a completed application in typed or printed form to Tammy Allison, COO, Mary Rutan Foundation, 205 Palmer Avenue Bellefontaine, Ohio 43311, no later than Friday, April 4, 2008.  The Mary Rutan Foundation Board requests that all applicable areas of the application be completed.

In addition to the application form the following items are requested:

1. Two written references from persons in healthcare positions, employers, high school/ college/university/technical/joint vocation school teaching staff.
2. A copy of most recent transcript of grades.
3. An official letter of acceptance to the chosen medical career field (acceptance to the institution does not necessarily signify acceptance to a specific medical career area).  An up-to-date transcript of grades will be sufficient for applicants having received a scholarship/loan as recent as 2007/2008 academic year.
4. A typed statement describing financial need.  Please describe financial need in a complete paragraph as the statement will be important to the Mary Rutan Foundation Board. 
5. A typed or printed summary of your past year, which could include academics, school related extracurricular activities, work experience, travel, athletics, etc, in high school, post high school, or college/university.  If you received a scholarship or loan in 2007, please include interesting experiences relating to your medical career studies.
6. A photograph for publication in the local media if select as a 2008/2009 recipient.  (Head shot only please)

Interviews With The Mary Rutan Foundation Board
Scholarship/Loan selection will be made on May 16, 2008.  Personal or phone interviews with a board representative may be required prior to that date.   Notification of the Scholarship/Loan decision will be made to all applicants by May 23, 2008.
